Is It Possible to Get Dry Eyes from ACCUTANE?

Absolutely. Here's information about ISOTRETINOIN and dry eyes. This information is straight off the PI, product insert. Not surprising, most patients don't read it.

 

Adverse Reactions/Side Effects

Most common adverse reactions (incidence ≥ 5%) are: dry lips, dry skin, back pain, dry eye, arthralgia, epistaxis, headache, nasopharyngitis, chapped lips, dermatitis, increased creatine kinase, cheilitis, musculoskeletal discomfort, upper respiratory tract infection, reduced visual acuity.

 

The most common side effects of isotretinoin capsules include:

  • dry lips
  • dry skin
  • back pain
  • dry eyes
  • joint pain
  • nose bleeds
  • headache
  • upper respiratory tract infection (common cold)
  • chapped lips or swelling of the lips
  • skin reactions
  • muscle problems
  • eye problems, including decreased vision

 

5.13 Ocular Abnormalities

Visual problems should be carefully monitored. If visual difficulties occur, discontinue isotretinoin treatment and obtain an ophthalmological examination [see Adverse Reactions (6)].


Corneal Opacities

Corneal opacities have occurred in patients receiving isotretinoin capsules and more frequently when higher drug dosages were used in patients with disorders of keratinization. The corneal opacities that have been observed in clinical trial patients treated with isotretinoin capsules have either completely resolved or were resolving at follow-up 6 to 7 weeks after discontinuation of isotretinoin [see Adverse Reactions (6)].


Decreased Night Vision

Decreased night vision has been reported during isotretinoin use and in some instances the event has persisted after therapy was discontinued. Because the onset in some patients was sudden, patients should be advised of this potential problem and warned to be cautious when driving or operating any vehicle at night.


Dry Eyes 

Dry eyes has been reported in patients during isotretinoin use. Patients who wear contact lenses may have trouble wearing them while on isotretinoin treatment and afterwards.

How ACCUTANE Alters Tear Film Balance

ACCUTANE works by reducing oil production in your skin’s sebaceous glands. Unfortunately, your meibomian glands on the eyelids respond the same way. Less oil means a thinner lipid layer on your tears. When that oily barrier weakens, tears evaporate more quickly, leading to EVAPORATIVE DRY EYE symptoms.

 

Recognizing Dry Eye Symptoms on ACCUTANE

You might feel burning or stinging. Your vision can fluctuate. Sometimes you tear excessively, a paradox caused by the inflammation. Understanding COMMON SIGNS helps you take early action.

Actionable Strategies to Combat ACCUTANE‑Related Dryness

Anyone on ACCUTANE should receive a detailed and personalized eye care routine from their DRY EYE SPECIALIST OPTOMETRIST. Here I outline my MGD Cleansing System as a good starting point. 

In addition to the below, supplement with preservative‑free artificial tears during the day. Make sure they are preservative-free. Consider adding omega‑3 fatty acids to your diet. These oral supplements can support healthier gland function over time.

 

1) Warm Compress & Massage: Heat is your friend. Use Focused Eye Roll for 30 seconds a day soften and slowly drain debris. Repeat 30 second sessions during the day as needed. Spread the sessions apart so the eyelid skin doesn't become damaged. For upper eyelid, roll down towards the eyelid margin. For lower lid, roll up towards eyelid margin. 

 

2) Gentle Eyelid Scrub: Add 1 drop of Tea Tree (brown label) or Rosemary (green label) Eyelid & Eyelash Cleansing Oil on Eyelid Margin Scrub Brush. Gently scrub along the eyelid margin to keep an exit site open for debris to drain. The body will want to heal itself. Without an exit site, the debris will likely pop through the eyelid and we don't want that.

 

3) Foaming Lid Cleanser: Finish the double cleanse Tea Tree (blue label) or SuperGLA (purple label) Eyelid & Eyelash Foaming Cleanser to wash away any drainage material.
